Prospective vendors must be prepared to provide the following:

• Current company contact details and owner/principal information
• Professional licensing documentation (if applicable)
• Specific properties where services will be offered
• General Liability insurance with Additional Insured wording
• Auto insurance documentation
• Workers Compensation insurance documentation
• Completed W-9 form
